Use this form to provide parent / guardian authorization for a ROOK (ages 12–17). Only a parent or legal guardian submits and manages ROOK participation; OWNTIME does not create youth accounts or public profiles.

Submitting authorization does not by itself create final participation approval, and individual activities may require their own permissions. A parent or legal guardian can end ROOK participation at any time by contacting OWNTIME.
